Tier-2 (NDA) Disclosure Request

Tier-2 disclosure is controlled and provided under NDA for legitimate due diligence needs.

  1. Requestor identity (legal entity, role, contact)
  2. Purpose (bank/regulatory due diligence scope)
  3. Requested artifacts (Doc IDs + versions)
  4. NDA status (executed / pending) and preferred signing flow
  5. Confirm: no patient-level data, no sanctions circumvention intent
